Info
Nothing unusual here. Place for the basic information about the character. GM is the name of the game master, Setting is what setting the campaign takes place in.
Abilities
The six standard abilities along with untrained scores and resistance modifiers. I leave places for all six resistance modifiers even though constitution and personality usually don't have any, I've seen them in third party products.
Last Resorts
Straight forward. You write your max points in the Points and darken boxes on the right until you have empty boxes on the left equal to that number, then you check off any used last resorts starting from the left. Cost is simply the cost in skill points to buy a last resort point.
Armor
Name of armor and resistance to each type of damage.
Action Check
The action check scores for marginal, ordinary, good, and amazing, along with any die step modifier, and the number of actions per round.
Game Data
Here we have special abilities (usually racial), up to 4 perks, and 4 flaws, and the attributes: motivation, attitude, and two traits.
Personal
This is personal information in that it does not tie into the game mechanics per say. Standard info such as age, height, etc. Allegiance is who the character is loyal to; could be self, a country, an organization, etc. Citizenship is what nation/country/planet the character is a citizen of, vs. Birthplace which is where they were born. SocStatus is the characters social status and could be descriptive or use the rules in the GMG.
Movement
Movement rates for all types of movement.
Achievement Track
An achievement track. Circle the current level and that shows how many points you need to reach that level. When a level is reached erase all checked boxes and add to the number of points erased to Stored. When spending points subtract points from Stored and add to Spent.
Portrait
A large blank area intended for a portrait of the character. Could also be used as additional note space, symbol, or anything else.

Durability
Write the maximum in the blank spaces, darken boxes on the right until there are a correct number of what boxes, put checks in white boxes as points are lost.
Weapons/Attack Forms
Large attack form box. Has the attack form, score, base die, accuracy, range, type of damage, damage (O/G/A), actions per round, clip type, hide modifier, mass, and any notes.
Ammunitions
Here is where you list the various types of clips you have, the number of clips, and the state of the current clip. Blacken boxes on the right to designate the size of the clip and then check off boxes as the clip is used. Clips with more than 20 shots can use multiple entries.

Both these sections are identical in layout: who, where, career, and any notes.

Wealth
A place to lift wealth with type, quantity, where the wealth is stored, and any notes.
Situation Die Step Scale
Lists the die at the stop and then modifiers for broad skill use (base +d4), specialty skill use (base +d0) and checks with a bonus (-d4 base).
Notes
Large section for notes.

The author version text. If this does not match what is on your sheet then this description is probably not quite accurate.
