Registration Form and Application for Funding

Deadline for funding consideration: March 15, 2009.

All participants must register for the workshop by completing the top portion
of the form below. Some funding for participants (particularly for graduate students and
recent PhDs) is anticipated. To be considered for funding, the lower portion of this form
must also be completed.

Recent PhD (received during or after 2006).
            List thesis title and advisor.  
PhD received before 2006.

Will you be able to participate in the conference which follows the workshop?

If so, you do not need to complete a separate registration for the conference.

for funding, please complete the following information.

Finally, if you are a graduate student or recent PhD, please arrange for one letter of
recommendation to be sent to conference@liegroups.org.  Your application for funding
will not be complete without the letter of recommendation.
